Foundation for Active Member Education (FAME)

The California Health Information Association’s FAME scholarships are awarded to provide opportunities for CHIA Active members to attend CHIA-sponsored educational programs in those instances when the member may not otherwise be able to attend due to financial limitations. It is the goal of CHIA to provide FAME funds to as many qualified applicants as possible annually.

FAME Award Criteria
Announcement of FAME funds availability and eligibility criteria will be published in the CHIA Journal (generally the December/January issue) and posted to the CHIA Web site.

FAME Award amounts The amount of the expenses covered will be determined on an individual case basis, taking into consideration the content of the application itself, the fund balance available, the number of anticipated applications, and the educational program details, and the amount of the funds requested.

The approval of the FAME scholarship award will entitle the recipient to free registration at a CHIA-sponsored educational event. Approval of the application may include approval for reimbursement for all or part of the expenses requested. Travel and lodging will be considered for reimbursement if the educational program location exceeds 100 miles from member’s home. Lodging is considered for shorter distances if the program is more than one day in length.

Eligibility for FAME Awards

Only CHIA Active members who have not received a FAME award are eligible to apply. CHIA Student members are not eligible for FAME awards. Recipients of FAME Awards are not announced to the CHIA general membership.

Application for FAME Awards

Any eligible CHIA Active member may apply for educational funds to attend a CHIA-sponsored educational event, by submitting a completed FAME application form to the CHIA Executive Office. Applications may be submitted anytime during the year, but must be received by CHIA at least 30 calendar days in advance of the educational event for which award funds are being requested.

Eligible members are encouraged to apply for their needed amount. However, when an application is approved, the funds approved may be for a portion of the requested amount.

If the seminar is part of a series, the applicant will only be considered for the seminar site closest to his/her geographic location.

You can download the FAME application form, view and print out. Submit completed application to CHIA,1915 N. Fine Ave., Suite 104, Fresno CA 93727-1565, or fax to (559) 251-5836, or e-mail to
FAME@CaliforniaHIA.org.

Approval of FAME Award applications

FAME applications will be considered for approval by the Executive Director and the CHIA President or other Board member designee(s).

Each applicant will be notified within ten working days whether his/her application has been approved or denied.

Contributions / Donations

As a non-profit educational association (IRS: 501-(c)(3)) contributions to the CHIA Scholarship Fund and FAME funds are tax-deductible. A suggested contribution of five ($5.00) dollars is solicited from the membership in conjunction with the annual dues assessment notice. Monies collected from the annual Silent Auction held at the State Convention & Exhibit also go to these two funds.

To recognize the generosity of individual members who contribute five ($5.00) dollars or more to the scholarship fund, CHIA will print the names of contributors in a CHIA Journal. Their name will also appear on the CHIA Web site. Those who donate to these funds may do so ’in honor’ or ’in memory’ of another individual, if they wish.

Donations will be deposited into the funds as designated by the donor. If there is no designation, the funds will be deposited equally in the established funds.

Donations will be accepted at CHIA Executive Office at any time throughout the year, including with annual membership assessment.
