Print your secure confirmation form that the application was sent to California Health Information Association (CHIA).
CHIA membership is contingent upon your current membership status with the American Health Information Management Association (AHIMA) (http://www.ahima.org/membership/).
Dues are calculated on an anniversary basis, matching the membership anniversary yar with the AHIMA. To start or renew your membership please complete this application form, CHIA strives to maintain accurate, up-to-date contact information for all our members. This helps us provide you with timely mailings, important information, and valuable services.

You can download and print out the application form on your own.
When completed you can either fax the application (with credit card information) to (559) 251-5836, e-mail to Membership@CaliforniaHIA.org (with credit card information), or you may mail the application with a check or credit card information to the CHIA Executive Office at 1915 N. Fine Avenue Suite 104, Fresno, CA 93727-1565.
